The optimizer performs lossless and near-lossless optimization. For PNG it re-applies zlib/DEFLATE compression with smarter filter selection per scanline, strips ancillary chunks (color profiles you do not need, editing metadata, timestamps) and optionally reduces the bit depth when the image uses fewer colors than the format allows. For JPEG it re-encodes the Huffman tables and removes EXIF bulk without ever touching the quantized pixel data.

Because the pixel data itself is not re-encoded in lossless mode, optimization is completely safe: the output is pixel-identical to the input, just packaged more efficiently. Near-lossless mode goes one small step further by cleaning nearly-invisible noise, which frequently unlocks another 5–10% of savings.
